1. Introduction

Since an erection requires a sequence of events, impotence can occur when any of these events are disrupted. The sequence includes nerve impulses in and from the brain, spinal column, and area of the penis, and response in muscles, fibrous tissues, veins, and arteries in and near the corpora cavernosal.
Damage to arteries, smooth muscles, and fibrous tissues, often as a result of disease, is the most common cause of impotence. Diseases -- including diabetes, kidney disease, chronic alcoholism, multiple sclerosis, arteriosclerosis, and vascular disease -- account for about 70 percent of cases of impotence. Between 35 and 50 percent of men with diabetes experience impotence.
Surgery (for example, prostate surgery) can injure nerves and arteries near the penis, causing impotence. Injury to the penis, spinal cord, prostate, bladder, and pelvis can lead to impotence by harming nerves, smooth muscles, arteries, and fibrous tissues of the corpora cavernosa. Also, many common medicines produce impotence as a side effect. These include high blood pressure drugs, antihistamines, antidepressants, tranquilizers and appetite suppressants. Experts believe that psychological factors cause 10 to 20 percent of cases of impotence. These factors include stress, anxiety, guilt, depression, low self-esteem, and fear of sexual failure. Other possible causes of impotence are smoking, which affects blood flow in veins and arteries, and hormonal abnormalities, such as insufficient testosterone.

2. Impotence is treatable

Most physicians suggest that treatments for impotence proceed along a path moving from least invasive to most invasive. This means cutting back on any harmful drugs is considered first. Psychotherapy and behavior modifications are considered next, followed by vacuum devices, oral drugs, locally injected drugs, and surgically implanted devices. In recent years the vacuum erection aid systems have gained more and more recognition since the vacuum therapy is considered as the least invasive option to treat ED and is often the only help for patient suffering from impotence.


3. Treatment with the Vacumat

Vacumat is an erection aid system with electronically controlled vacuum build-up leading to an increase in quality and comfort of the erection. The non skin-irritating silicone cuff is placed over the acrylic cylinder using the guiding cone (Instruction for use, Illustration 9). The penis is placed into the acrylic cylinder. After the electronically controlled vacuum pump is connected, air will be drawn out of the cylinder and blood into the penis, causing an erection. The increase of vacuum is constantly controlled not to over-exceed a pressure of 0.4 bar, significantly reducing the risks of a haematoma. The soft silicone cuff is now placed around the base of the penis. The cuff will maintain the erection after the cylinder is removed and during intercourse by preventing blood from flowing back into the body. Following intercourse or longest after 30 minutes the silicone cuff will be removed allowing the blood to leave the corporal bodies and thus creating a flaccid penis again.


4. Silicone Cuff- the significant difference

Patients using other vacuum devices often complain about delayed ejaculation. Tension rings placed around the base of the penis to hold the blood in the penis are too tight to allow a free semen flow. As a result semen is only expelled after the tension rings have been removed. A soft silicone cuff placed around the penis significantly reduces the risk of delayed ejaculation as well the occurrence of haematoma.


5. Effectiveness

Clinical studies have been conducted with a large patient group documenting the successful use of vacuum erection aid systems. Patients have reported a significant improvement in the ability to have an orgasm, the erection quality and the sexual intercourse frequency. The enhancement of sexual satisfaction and general self –esteem is widely reported as well. Turner et altera report that a daily use of vacuum devices without applying the silicone cuff can lead to an improvement of the erection quality and size increase of the penis in girth and in length.

7. Assembly of the system

Assemble the system as illustrated:
1. Vacumat elements.
1.1 Switch to activate the pump.
1.2 Battery lock.
1.3 Ventilation valve

Note.
Make sure, that a fully charged 9-volt battery is inserted. To change the battery, remove the cover of the battery compartment using the battery compartment key, replace the battery and reattach the cover.

8. Using the Vacumat

2. Placement of silicone cuffs
2.1 Place the acrylic cylinder on flat surface and attach the cone and "slip of”- ring

2.2 Remove the silicone ring from the case and inspect the ring for its undamaged condition

2.3 Lubricate the cone surface and the opening of the silicone cuff with the lubricating gel.

2.4 Slip the silicone cuff over the cone until it reaches the \"slip of”- ring

2.5 Remove the cone. In order to form a better sealing of the silicone cuff at the body, place a few drops of the lubricating gel on the front side of the silicone cuff.

2.6 Attach the vacuum control unit to the acrylic cylinder

Vacumat is now ready to use.

9. Use of the Vacumat with the silicone cuf

Place the penis into the acrylic cylinder. Hold the device firmly against the body to form an air seal.

Switch on the vacuum control unit. This creates a vacuum around the penis, causing the body's blood to enter the corpora cavernosa, engorging the penis in a way similar to a natural erection.

The vacuum build up and forming of an erection should be completed within about 50 seconds. Make sure that the cylinder remains firmly pressed against your body during the vacuum build up.

Note: In order to avoid any haematoma the maximum vacuum is controlled to reach a maximum of 0.5 bar.

Hold with one hand the "slip of”- ring pressed against your body and with the other hand move the cylinder away from your body by carefully rotating the cylinder. This movement will place the silicone cuff over the erected penis.

Should there be any problem when removing the acrylic cylinder because of the vacuum surrounding the penis, push for a brief moment the red ventilation button positioned on the front side of the vacuum control pump.

The placement of the soft silicone cuff around the penis shaft ensures the stabilization and firmness of the erection. The expelling of semen during ejaculation will not be affected.


10. Removal of the silicone cuffs.

Place two fingers between the penis and the silicone cuff and move the cuff carefully forward over the penis.


11. Daily use of the Vacumat to improve erection quality over time

Multiple usage of the Vacumat as a "training tool” can lead to an improvement of the erection quality. Over time the penis may increase in length, girth and rigidity.
1. Place the penis into the acrylic cylinder.
2. Hold the device firmly against the body to form an air seal.
3. Turn on the vacuum control unit. This creates a vacuum around the penis, causing the body\'s blood to enter the corpora cavernosa, engorging the penis in a way similar to a natural erection.
4. Hold the erection for several minutes
5. Push the ventilation valve button to completely eliminate the vacuum
6. Repeat the procedure several times

12. Helpful tips and recommendations

Make yourself and your partner familiar with the Vacumat. Since experience has shown that best results are achieved with a more moderate vacuum, it is advisable taking the time to determine your own individual vacuum dosage.
The front part of the silicone cuff is very delicate. Do not use fingernails or sharp objects to manipulate the silicone cuff. Use generously lubricating gel, when placing the cuff over the cone.
Important: Placement of the silicone cuff on the cylinder should never last more than 5 minutes; silicone attached to plastic/acrylic can cause adhesion and the removal might become difficult.
In case you experience difficulties removing the silicone cuffs from the penis, squeeze the penis causing the blood to evacuate the corporal bodies completely and form a better flaccidity. The silicone cuff can now easily be removed.
The company cannot be held responsible for damages as a result of inappropriate handling. Often asked question: Is there a difference between a natural erection and a \"vacuum” erection? Answer: An erection generated by a vacuum will lead to higher rigidity since the vacuum will cause the complete filling of the corporal bodies with blood.
An erection can last longer; beyond the ejaculation.
Cleaning of the Vacumat.
Clean the parts (excluding the vacuum control unit) with warm (not too hot!!!) soap water. The vacuum control unit can only be cleaned with a moist cloth/towel.
Make sure that no water will enter the vacuum control unit.

5. The inserted 9 V battery lasts for approximately 50 usages.
5.1 Open the battery compartment using the battery compartment key
5.2 Remove the cover
5.3 Remove the old battery (use the key to lift the battery)
5.4 Connect the new battery
5.5 Reattach the cover
